PR Form
  1. Purchase 24-lb. legal size paper (8.5"x14") in WHITE.
    Standard weight (20-lb.) copy paper (8.5"x11") will NOT be accepted.
  2. Download the PR form.
  3. You will need the Adobe Acrobat Reader Plug-in. You can download the software for free from the Adobe site.
  4. Complete the PR form.
  5. Legal size paper must be selected from the page setup menu prior to printing. Check to make sure no "shrink to fit" or "reduce size" options are applied or the PR form may not print correctly.
  6. Print the completed PR form.
  7. The associate member must verify the information is correct and sign the PR form.
  8. Affix the chapter seal.
  9. Repeat steps 3-7 for each associate member.
