STOP! Follow these five steps to avoid a $25/man fine this semester.

1. Make sure your “D” has paid each initiate’s Associate Member Dues (currently $100/man).

2. Complete this form (typed) for each member to be initiated and save a copy for your records.

3. Print the completed PR form and have the member sign the bottom line. (24lb 8.5” x 11” paper is preferred)

4. Mail all completed PR Forms into IHQ along with a check for Initiation Dues (currently $205/man) at least TWO WEEKS PRIOR to your intended initiation date.

5. Once all paperwork is received and processed, you will receive a notice of approval. You may then initiate all approved men.

If you do not receive approval, you will be fined.
